Affidavit Details

All applicants for a Employee/Independent Salesperson License must include an Affidavit sworn to before a notary public, by an Employee/Salesperson located in and licensed to engage in the Employee/Salesperson Business in the District of Columbia, certifying that the applicant will be employed by a Contractor and Construction Services Business (i.e., General Contractor/Construction Manager, Home Improvement Contractor) or a Vehicular Services Business (i.e.,Motor Vehicle Dealer, Parking Facility, etc.).
