Financial Report for Employment Services Details

All applicants for a Basic Business License for an Employment Services Business must submit a Financial Report Form for Employment Services that details information regarding the financial condition of the Employment Agency business. A notarized financial statement of the persons forming the business may be used to satisfy this requirement. Additionally, a signed permission form for the Department to request a credit check must be provided.
