All applicants for a Basic Business License for a Regulated Business (Motor Vehicle Dealer) must provide blank preprinted sample sets of the Car Order and Bill of Sale and/or Conditional Sales Contract containing the organization’s name, premise address, and telephone number. The forms must be typewritten and submitted in triplicate.
